Children's Aid Society of Utah offers services to families pursuing adoption through attorneys or other agencies. 

PRE-ADOPTION EVALUATION:
·      $425 Application Fee
·      $800 Pre-Adoption Evaluation Service Fee
·      Applicants who reside beyond a 100-mile radius of the Agency's office in Ogden will be charged an additional fee for travel expenses incurred by the caseworker in order to complete the Pre-Adoption Evaluation.

The Pre-Adoption Evaluation reflects facts obtained through written information provided by the applicants, personal interviews with the applicants, and information obtained by the agency from medical professionals and references.  It will also contain Children's Aid Society's caseworker's impressions and a recommendation from its Adoption Committee.

Upon completion of the Pre-Adoption Evaluation, Children's Aid Society will send a copy of the Evaluation to the agency or attorney facilitating the adoption.  In the event that the agency is requested to send additional copies to other agencies, a $10.00 fee per copy will be charged.

POST-ADOPTION EVALUATION:  Post-Adoption Evaluation services are also available for a fee of $400.00, plus mileage fees, if applicable. This service includes three post-adoption evaluation visits during the six-month period after placement prior to legal adoption, and appearance at the adoption finalization hearing by a Children's Aid Society representative.

OTHER SERVICES: Designated adoption services are available to adoptive parents and birth parents who have selected each other through other means but desire agency adoption services. 

 Pre-adoption and post-adoption counseling is also available to birth parents who place through other sources. Fees for these services are paid by the adoptive family or the individual responsible for placement.
